The Blood of Dawnwalker doesn't have a ton of romance options throughout the game, but the few that are included are absolutely worth pursuing if you're interested in delving deeper into the game's story and characters. While Coen has a unique relationship with many NPCs in Vale Sangora and can even express his undying love for Anca, his relationship with Lacra is absolutely one of the most compelling and complicated in the game.
!!! WARNING: THIS ARTICLE CONTAINS SPOILERS FOR THE BLOOD OF DAWNWALKER !!!You'll first encounter Lacra fairly early in your 30-day run, assuming you're following the main quest lines to some extent. While she may come off as antagonistic and uninterested in any sort of romance at first, this changes rather suddenly while pursuing her main quest.
Coen will have to choose a handful of specific dialogue responses in order to unlock a very spicy sex scene with Lacra, and while it isn't necessary to complete Lacra's quest, it does show a totally different side of her character. Further, pursuing a romance with Lacra will not affect your ability to romance any other characters in the game.
You Can Romance Lacra During The Night Of Horrors

Once you progress Lacra's storyline and have completely gone through Isbrand's memories, you'll have to wait a day or so before returning to her and eventually starting the final leg of her questline, "The Night of Horrors." During this quest, you'll follow a group of wisps through various groups of enemies and eventually end up finding a sarcophagus and enter a hidden cave.
Inside, you'll end up triggering a cut scene where you learn the tragic fate of Kallius, but also discovering an unexpected surprise Mandrake. Lacra will explain to Coen exactly what this is, and you'll have the option to either eat the Mandrake or not, but in order to romance Lacra you MUST eat the mandrake, otherwise the scene night ends quickly and unceremoniously.
How To Romance Lacra

Once you've eaten the Mandrake, a romantic scene with Lacra is still not assured unless you make the right dialogue decisions. After the Mandrake's power courses through both Coen and Lacra, you'll need to "Follow Lacra" and then have a short conversation about Coen's family. She will then slice open her wrist and invite Coen to "have some fun," and you'll need to drink from her wrist.
After drinking her blood and biting her neck, you'll need to select the "Kiss Her" option and brace for some X-rated content. Once Lacra and Coen get done slashing and biting through their violent sexcapade, the scene will end, and Coen will wake up alone on the cavern floor.
What Happens If You Romance Lacra?
While you don't necessarily have to romance Lacra in order to unlock it, eating the Mandrake will give Coen a new Vampire power: Mandrake Ward. This allows you to avoid dying once a night after taking lethal damage, automatically restoring three health segments. This can be unlocked even if you decide not to romance Lacra, and isn't necessarily all that useful unless you are playing on higher difficulty.
Further, players will unlock the Mesmerise ability in the Vampire skill tree. This can also be unlocked whether you decide to romance Lacra or not, but if you choose not to go the spicy route, you'll acquire a vial of Lacra's blood that can be consumed to unlock the ability instead. Additionally, completing The Night of Terrors quest will unlock the Misery Loves Company trophy.
Overall, none of these rewards are dependent on actually romancing Lacra, and the only real bonus players get in The Blood of Dawnwalker is the additional sex scene. You won't be able to romance Lacra again outside of this encounter, though you'll be able to reminisce on the night should you decide to pursue the final battle against Brencis alongside her.
