Real Housewives of Atlanta star Porsha Williams has revealed she underwent an emergency partial hysterectomy after doctors found 21 fibroids in her uterus.
Williams, 45, who is mom to daughter Pilar, seven, detailed how the fibroids, including one the size of a cantaloupe, were discovered when she asked the doctor for an exam while accompanying a friend to a fertility appointment about freezing her eggs.
In an essay for Glamour Magazine, Williams said the removal of her uterus 'saved her life' but that she would forever grieve the loss of her ability to have more children and carry a baby.
She said of the exam: 'The moment I got on the table, she went quiet, her face flushing. I asked what was wrong. “Hold on,” she said, “I’m trying to see something.” On the screen, all I could make out was blackness—then these huge white circles started appearing.
'She told me she wanted to send me to a fibroid specialist right away. She said she could see the fibroids had come back—she guessed three or four, though that number would turn out to be way off—and had me scheduled for an MRI by 2:30 that same afternoon.
'A few days later, she called with the results: a little over 15 large fibroids in my uterus, plus endometriosis and another diagnosis involving the uterine lining. As we sat in her office, she told me I needed a hysterectomy.'

Williams had previously been diagnosed with fibroids before her pregnancy with Pilar at age 30.
She said the fibroid 'outgrew' her first pregnancy and caused a miscarriage at four months.
After delivering Pilar, who she welcomed with ex-fiance Dennis McKinley, the fibroids returned and she underwent embolization – a surgical treatment that shrinks noncancerous uterine tumors by blocking their blood supply.
Williams had a partial hysterectomy, which allowed her to keep her ovaries, and said: 'My quality of life has improved 100% since. I hadn’t realized the fibroids were behind my migraines, my low iron levels, or how stressed my body had been just from everyday life.
'I’d also lost my ability to have sex, since the fibroids had grown large enough to close off more than 60% of my birth canal, and put my bladder at risk from their sheer weight.'
Williams said she is now coming to terms with the fact she will not be able to have more children, adding: 'I always wanted four kids. I have three siblings and always pictured a big family of my own.
'That first diagnosis and miscarriage were the first signs it might not happen, but after Pilar, I did start to imagine having one more. I’m an older mom, but I had the right support system and financial stability. I felt optimistic.'
Last month, Williams scored a major win in her divorce settlement from Simon Guobadia.

The television personality and singer, who split from the Nigerian-American businessman in February 2024 after 15 months of marriage, walked away with a seven-figure payday and the marital home, according to court documents obtained by the Daily Mail.
Williams secured sole ownership of the couple's $7 million Atlanta mansion after Guobadia transferred his interest in the property to her via a quitclaim deed.
Williams, who revealed the identity of her new girlfriend via Instagram last November, also received a $1,568,181.63 reimbursement of Guobadia's share of the equity in their home, along with the title to a 2021 Rolls-Royce Ghost, per court records.
The deed shows that Williams now has the authority to sell or refinance the 15,284sq ft, five-bedroom, eight-bath home without needing his consent or the intervention of an arbitrator.
Back in June 2025, Williams admitted to People that her acrimonious split from Guobadia has forced her to reconsider her approach to relationships.
The R&B artist, who returned to The Real Housewives of Atlanta in March of last year after a hiatus, also confessed to being a 'people pleaser.'
'I used to only see myself through men's eyes, and that's why I went through a lot of abuse with men in my twenties,' she shared. 'And then when I got on reality TV for the first three years, I only saw myself through what the fans would say in the comments. I was like a walking culmination of what the comments thought.
'But now, I could give zero you-know-whats. I'm my own person. I took charge of my life, financially, in my relationships — everything. I'm really confident now. But something like this, it shakes your confidence, you know? Because you work so hard and you think everything is going one way and when it doesn't work out, it's disappointing.'

Williams said she was determined to fully heal before embarking on a new romance.
'I can't just feel it with someone else. I can't just get underneath someone else and feel it. I can't just drink it away like I have in my past,' she said at the time, adding: 'It's a rough road, healing this way. It's dry — on every level of being dry. But it's needed.'
Not long after she made these statements, Williams chose to pursue a relationship with Patrice 'Sway' McKinney.
The reality star and the salon suite entrepreneur met via Instagram in 2018 but reconnected romantically years later and went Instagram official in November 2025.
WHAT IS A HYSTERECTOMY?
A hysterectomy is a surgical procedure to remove a woman's uterus.
There are three kinds:
- PARTIAL HYSTERECTOMY: Removes two-thirds of the uterus.
- TOTAL HYSTERECTOMY: Removes uterus and cervix.
- RADICAL HYSTERECTOMY: Removes uterus, cervix and vagina.
The operation is most commonly performed on women between the ages of 40 and 49.
More than 20 million American women have had a hysterectomy, according to data from the Centers for Disease Control and Prevention.
As women approach menopause, the odds that they will develop one of several serious uterine health conditions increases. Doctors may recommend a hysterectomy as a treatment for:
- fibroids
- endometriosis
- uterine (endometrial) cancer
- chronic uterine pain or bleeding
- collapsed uterus
In some cases, doctors may suggest a hysterectomy as a preventative measure if a woman has significant warning or early signs of developing one or more of these conditions.
When necessary, surgeons may also remove the ovaries and fallopian tubes, if these have also been damaged or are at serious risk of damage.
The removal of reproductive organs sends a woman's body into menopause, no matter how old she is.
This comes with unpleasant side effects like hot flashes, and many women have to start hormone therapy, taking estrogen to balance out their own hormones.
